Assessment Binders/Assessment Folders

Setting up an organized system for storing your student's assessments and data will save yourself lots of time (and sanity) in the long run. 

That’s why I created these assessment binders!

Each of my students that are on RSA, being progress monitored, on a RTI plan, etc. have their own personalized assessment binder.

I store all their assessments and data in their own individualized binder.

It makes it easy to pull each time I need to document their progress, fill in important documents, etc.

I also label the spine of the binder so I can easily see whose binder I'm pulling.

If you don't want to create each student their own personalized cover for their assessment binders, you can use these pencil people covers!

I've also added covers to make your own Assessment FOLDERS!

Simply pick a kid character for each of your students.

Type in their name, student number, and student info.

Print, laminate, and attach the covers to the plastic folders.  You can attach the covers to the front of the folders by stapling them or using Velcro dots.

Store your assessment folders in a basket or bin!

Use the pencil people covers to create a standard cover for your assessment folders.

SIGHT WORD FLUENCY PHRASES AND SHORT SENTENCES ASSESSMENTS

If you are looking for a new sight word assessment for your students check out my Sight Word Fluency Phrases and Short Sentences Assessments.

In the past I used my Single Word Sight Word Assessments for most of my first grade RTI kids that struggled mainly in the area of reading fluency.  You can read more about those assessments HERE!

But if you are looking for a step above single word sight word assessments then check out these Sight Word Fluency Phrases and Short Sentences Assessments.

These phrases are a wonderful way to test students on how well they are building and improving their sight word recognition and reading fluency.  These assessments are great to use for intervention assessments such as RTI, progress monitoring, etc.

Send home the assessment pages so parents can see how their child is doing.

How it Works: Each assessment is color coded based on the Dolch word list used.

For example:

Blue Set: Pre-Primer Words

Orange Set: Primer Words

Yellow Set: 1st Grade Words

Purple Set: 2nd Grade Words

Green Set: 3rd Grade Words

Many of the high-frequency words are used multiple times and each set of phrases builds on one another.  For example, the orange set has words from the pre-primer and primer list of sight words.  The yellow set has words from the pre-primer, primer, and 1st grade list of sight words, etc.

Each version has student reading pages and teacher recording pages.

Pick the assessment version that fits your needs.  Decide if you want to assess 25, 38, or 75 phrases and/or short sentences.  

Use the student data recording pages so you can track your student's data and progress throughout the school year.

Sight Word Assessments for RTI/Progress Monitoring

After a couple years of trial and error on different assessments and things I could do for my RTI kids I finally found one that was easy to implement and very successful for my RTI students.

For most of my first grade RTI kids they struggle mainly in the area of reading fluency.  Therefore, the primary focus of their interventions are with sight words.

For each of my RTI kids I assess them once a week (usually on Monday) using these sight word assessments I created.

I call them over to my table one-by-one during center time and have them read the words off of the assessment list.

  

While they are reading the words off of the list I record how they did on my data sheet.  I record which words they read correctly with a check mark and which words they miss with an X.

After giving the assessment I then take five words that they missed on the assessment and create a rapid word recognition chart.

The chart takes those five sight words and mixes them up in a different order on each line.

Then during the week I have each of my RTI kids practice that chart with me one-on-one.  I usually pull them at least 3-4 times a week during center time or computer lab time to run through the rapid recognition chart with me.  

I can easily see progress being made each day as they get better and better at reading the words and also faster and faster.

I follow this same routine every week.  Usually by the next assessment the student has mastered those five sight words they had been working on through the week and I add five new words for them to practice after their new assessment.

Through the weekly assessments you are able to see progress as the number of sight words they have mastered increases and increases.

As they continue to master more and more sight words they can move up to a different set of words.

*For my really low students I have letter sound and CVC blending assessments that I created that I follow basically the same routine with.  I only use those assessments on my really low kids that need to master those skills before being able to practice sight words.

I also have these sight word flash cards (that align with the sight word assessments) that I have my students practice with.

These sight word phrases and short sentences flash cards are also great for my students that are a little more advanced.
